Data accessibility viewed as key to closing the gap between value-based care and health IT 

​Although much progress has been achieved in digitizing the healthcare system, today's health IT infrastructure still struggles to support a transition to "value-based care," according to a new report from the American Medical Informatics Association.

The report, titled, "Crossing the health IT chasm: considerations and policy recommendations to overcome current challenges and enable value-based care," outlines policy recommendations for patients, providers and researchers to improve access to healthcare data for all the groups and proposes new standards for IT systems and applications.

Report shows registries advancing patient care for heart patients  

Two doctors inserting stent Registries continue to improve the effectiveness and safety of patient care of heart disease in hospitals throughout the U.S.

A recent report shows registry data is providing unique perspectives into the care and outcomes of heart disease and outlines the strength of national quality programs, like NCDR, in advancing the effectiveness of patient treatments.

Published in the Journal of the American College of Cardiology, the study addressed trends in heart disease care from the American College of Cardiology's National Cardiovascular Data Registry.

The authors examined patient populations, participating centers and patterns of care based on 2014 data from four of the 10 NCDR programs -- CathPCI Registry, ICD Registry, ACTION Registry-GWTG and IMPACT Registry.

"We were able to report on the care and outcomes of patients undergoing common cardiovascular procedures, including percutaneous coronary interventions, implantable defibrillators, and interventions for congenital heart disease as well as for patients with acute heart attacks," NCDR Management Board Chair and lead author of the study Frederick A. Masoudi, MD, MSPH, FACC , said. "In some cases, hospitals are consistently providing excellent care; the registries, however, allow us to identify those aspects of care where the cardiovascular clinical community can improve."

Healthcare providers tackle data security issues 

The proliferation of cyberattacks on healthcare providers is well known, with new reports continuing to highlight the problem.

More than 216 hospitals were included in 1,798 breaches between Oct. 21, 2009 and Dec. 31, 2016, according to a report last week in The Journal of the American Medical Association. Additionally, 33 hospitals, or 15 percent, reported more than one breach. Of the 141 affected acute care hospitals, 52 were major academic medical centers.

Also, about 20,000 patients were affected in 24 of the 216 breached hospitals, and six hospitals had over 60,000 breached patient records.

Another recent report found that ransomware attacks more than quadrupled in 2016, with nearly half happening in the healthcare sector. These types of attacks are projected to double again in 2017, Beazley Breach Insights reported.

Some efforts are underway to form a coordinated response to this problem.

At a hearing last week to address cyberattacks in the healthcare industry, the House Energy and Commerce Subcommittee on Oversight and Investigations, Terry Rice, VP of IT risk management and CISO at Merck, indicated cybersecurity has become a top concern for healthcare organizations.

While hundreds of millions of health records have been compromised in data breaches in recent years, the extent of the problem may be inadequately reported. “Unfortunately, I believe these incidents underrepresent the risks we are facing as an industry,” Rice said.

To fight cyberattacks, Congress should provide organizations tax breaks for Information Sharing and Analysis Centers, educate the industry on the importance of information sharing, protect data shared through ISACs and advocate for public-private partnerships, Denise Anderson, president of the National Health Information Sharing and Analysis Center told the lawmakers.

“It’s become increasingly apparent that the industry needs a government representative who understands cybersecurity issues, threats, vulnerabilities and impacts, as well as the blended threats between physical and cybersecurity,” said Anderson.

Leapfrog List Puts Focus on Patient Safety 

Patient safety is once again in the news with the recent release of the Leapfrog Group's Fall 2016 Hospital Safety Grade List. Almost all the hospitals on the list received a passing grade. Of the 2,633 hospitals evaluated, 844 earned an "A," 658 earned a "B," 954 earned a "C," 157 earned a "D" and 20 earned an "F."

Leapfrog's biannual program assigns A, B, C, D and F letter grades to the hospitals surveyed. When compared to previous lists, several states showed significant improvement this time. North Carolina, for example, climbed to No. 5 in this fall's list, up from No. 19 in spring 2013.

Hawaii ranked No. 1 for the first time, while Alaska, Delaware, and North Dakota, along with Washington, D.C., brought up the rear. None of the bottom-ranked states had a hospital that earned an A grade.

Improving patient safety is, of course, a major priority for healthcare providers. Research published in The Journal of Health Care Finance found that medical errors cost the United States $19.5 billion in 2008 alone. A 2016 study estimated that these mistakes cause 251,000 deaths a year in the U.S., where they are the third-leading cause of death (after heart disease and cancer).

Early Reaction to MACRA Rule Mostly Positive 

Last weekend was a busy one for those trying to parse the new MACRA rule released on Friday. At 2,202 pages, the Medicare Access and CHIP Reauthorization Act rule wasn't exactly beach reading, and it gave the health IT community plenty to talk about on social media and in policy statements.

The dust is still settling, but it appears that early reaction to the rule was mostly positive. Healthcare organizations praised the CMS for being responsive to concerns they had raised during the comment period leading up to the rule's finalization. In fact, about 80 percent of the 2,000+ pages are comments CMS received and its responses.
The American Medical Association was pleased with the permanent elimination of the Sustainable Growth Rate (SGR) formula. "The new law," according to the AMA's press release, "gives many physicians the opportunity to be rewarded for the improvements they make to their practices and for delivering high-quality, high-value care to Medicare patients."
Other features that drew favorable reactions included:

  • The rule's overarching theme that improving the organization and payment models for medical care must stress quality over quantity.
  • Greater reporting flexibility for clinicians, as well as support for innovation in the delivery of care.
  • The formal adoption of a transition year during 2017, which makes major changes to the Quality Payment Program (QPP) reporting requirements, and provides a longer time frame for those transitioning to the QPP.
  • Emphasis on helping clinicians educate themselves about the rule.
  • Easing of the policy defining the Advanced Alternative Payment Model (APM), which will allow additional programs to quality.

But the rule is not without its detractors. "It's disappointing that the flexibility provided for quality reporting in 2017 largely disappears in 2018 and beyond," the Medical Group Management Association said in a policy statement.
Other organizations complained that the nominal risk standard defining the Advanced APM remains too high.

Improving the Business Performance of Your Heart Hospital 

An effective CVIS strategy can improve the business performance of your hospital

The primary goal of any healthcare provider is to improve the lives of patients through effective treatment. However, because they are also businesses, hospitals have concerns that entail much more than this. To be viable in the long term, hospitals must manage their margins to fund their mission.
There are three main pillars of business concern for any hospital:

  • Clinical—health outcomes are measured with the goal of healthier patients leaving the facility.
  • Financial—the dollars must add up to keep the enterprise solvent.
  • Operational—staffing and facilities are measured against cost and need.

New Operational Realities

Payers' shift away from a fee-for-service model toward a value-based payment model demands that clinicians and administrators expand the above-mentioned pillars to include cost, patient outcomes, and patient satisfaction.

These changes aren’t easy. Providers have long been paid based on quantitative measures: the number of procedures performed. New operational realities demand new ways of measuring the qualitative value of those procedures. Reimbursement is linked to these metrics, and hospitals must find ways to leverage their investments in data technology in order to maximize their financial opportunities.


Granular Data Brings Actionable Insights

Data is critical to the shift to VBP. For example, if we know that extubation within six hours improves patient outcomes, it makes sense to monitor that metric internally on an ongoing basis. When outliers crop up, data points gathered from across the treatment spectrum can allow us to understand why. Perhaps a different treatment was needed at the outset, or some other patient health factor influenced that measure.

Over time, granular data can allow us to understand which type of treatment is best for patient outcomes in that circumstance.

It is discrete, granular data that can help providers fine-tune their processes in order to improve patient outcomes—and of course patient satisfaction. This same kind of close analysis can be applied to reducing costs. But for all three new, expanded pillars, efficient data collection, management, and analysis are needed.

Remember, value equals cost / quality, so lower cost increases value and higher quality increases value. The U.S. healthcare industry is spending a lot of time looking at value in recent years. Patients, too, are slowly shifting to an awareness that they need to measure the cost of care against the quality of care.

Better targeting of care pathways and therapies will reduce variations in care and make the whole industry more efficient. By reducing costs and improving quality, providers and payers who embrace the new technologies will attract patients and payers looking for value.

A: Evidence from labs, meds, genomics and related biometrics will lead to more personalized medical care.

A: The keys to financial improvements in healthcare come from making efficient use of resources--from supplies to provider time, and from reducing variations in care due to identifiable variations in health status. The keys to success are understanding labor dynamics, making sure that the major care pathways are well defined and efficient, and managing outlier cases effectively.
